1. New Drugs and Indications

Finerenone (Kerendia) for CKD with type 1 diabetes — 16 September 2026

  • What: a non-steroidal mineralocorticoid receptor antagonist, approved for CKD with T2D since 2021; now extended to type 1 diabetes — the first new treatment for this group in over 30 years.
  • Evidence: FINE-ONE phase 3 (n=242): UACR 28% lower than placebo at 6 months; hyperkalemia 10.1% vs 3.3%, discontinuation for hyperkalemia 1.7%. NEJM 2026;394:947.
  • What it means: a new option to slow progression if you have T1D kidney disease; potassium must be monitored; not for dialysis patients (very low eGFR excluded).

Atacicept (Trutakna) accelerated approval for IgA nephropathy — 7 July 2026

  • What: a fusion protein blocking both BAFF and APRIL B-cell survival factors; weekly subcutaneous injection.
  • Evidence: ORIGIN 3 phase 3, proteinuria 42% lower than placebo. Accelerated approval on a surrogate endpoint; confirmation of eGFR benefit still required.
  • What it means: IgAN is the most common primary glomerulonephritis in Asia; this joins sparsentan, iptacopan and sibeprenlimab as targeted options. Not yet reimbursed in Taiwan.

Iptacopan (Fabhalta) converted to full approval for IgAN — July 2026

  • What: oral complement factor B inhibitor; accelerated approval Aug 2024, traditional approval July 2026 on two-year APPLAUSE-IgAN eGFR data.
  • What it means: proteinuria reduction translated into kidney-function protection — a step up in evidence. Meningococcal vaccination required (class risk).

Obinutuzumab (Gazyva) for childhood-onset idiopathic nephrotic syndrome — 25 September 2026

  • What: anti-CD20 antibody; approved for patients ≥2 years with frequently relapsing or steroid-dependent disease in complete remission, to reduce relapses (about half the relapse risk in the trial).
  • Also: Priority Review for primary membranous nephropathy, FDA decision expected November 2026.
  • What it means: a possible route to less steroid exposure for steroid-dependent nephrotic syndrome.

Semaglutide (Ozempic) — 2026 kidney analyses

  • FLOW (2024) showed a 24% reduction in major kidney events and 20% in all-cause death in T2D + CKD. 2026 analyses: protection consistent regardless of cardiovascular history (JACC 2026), improved quality of life (ERA 2026), effective across CKD severity.
  • What it means: for T2D + CKD, GLP-1 RAs are now part of kidney protection, not just weight loss; evidence in dialysis patients remains insufficient.

2. Guideline Updates

KDIGO 2026 Anemia in CKD (January)

First full revision in 14 years. Key points:

  • Broader work-up: reticulocytes, CRP, B12, folate, TSH at initial evaluation.
  • New terms: "absolute iron deficiency" → "systemic iron deficiency"; "functional" → "iron-restricted erythropoiesis".
  • Dialysis iron: proactive IV iron (after PIVOTAL), hold when ferritin >700 ng/mL or TSAT ≥40%.
  • Hb ceiling: keep <11.5 g/dL on ESA (stroke/thrombosis risk).
  • HIF-PHIs (roxadustat, daprodustat etc.): second-line oral alternative for ESA hyporesponse; no combination with ESA; stop if no response in 3–4 months.
  • Transfusion: symptom-driven and transplant-protective, not number-triggered.
  • What it means: if your doctor orders extra labs or changes your iron regimen, this guideline is likely why.

KDIGO AKI/AKD draft (March public review)

Final version expected 2027, integrating acute kidney injury and acute kidney disease.

3. Devices and Technology

Next-generation Tablo home hemodialysis (Feb FDA 510(k))

Outset Medical's upgraded Tablo is the first dialysis system to meet the FDA's latest medical-device cybersecurity guidance; shipping from Q2 2026. Not available in Taiwan yet, but the trend toward smaller, connected home HD continues.

NeoKidney portable artificial kidney (Netherlands trial from January)

Suitcase-sized, dialysate regeneration, 4.5 L water per session; ~50 patients at UMC Utrecht using it at home, targeting CE certification 2026–2027, results in 2027. See Beyond Dialysis.

4. Xenotransplantation

The EXPAND trial (United Therapeutics UKidney, 10 gene edits) performed its first transplant at NYU Langone in November 2025; by design a 12-week observation precedes the second, with six patients at two centers in cohort one. As of verification, no full first-cohort results have been published; 2026 reviews in Frontiers in Nephrology and PMC describe 2025–2026 as the start of the "clinical trial era". eGenesis's FDA-cleared trial is also under way.

What it means: a historic year, but still far from routine care; trials are limited to US patients aged 55–70 with poor waitlist prospects.

5. Practical Advice for Patients in Taiwan

  1. New drugs: TFDA approval and NHI reimbursement for atacicept, iptacopan and finerenone (T1D) typically lag the US by 1–3 years; ask about trials or named-patient import if you qualify.
  2. Anemia: if your ESA dose keeps rising with poor response, ask your doctor to evaluate iron-restricted erythropoiesis and inflammation per KDIGO 2026 rather than simply increasing the dose.
  3. GLP-1: T2D + CKD patients can discuss semaglutide's kidney benefit; dialysis patients should not self-start — evidence is insufficient.
  4. Don't be sold a "2026 breakthrough": every item here has a formal source; anything that charges fees, lacks a trial ID, or promises "no more dialysis" is not on this list.

Polycystic Kidney Disease (ADPKD): Inheritance, Predicting How Fast It Progresses, the Science Behind Tolvaptan and 3 Litres of Water a Day, Family Screening, and Whether to Remove the Kidneys Before Transplant

ADPKD is the most common inherited kidney disease — about 1 in 1,000 people carry the gene and it accounts for 5–10% of the world's dialysis population. PKD1 vs PKD2, how the Mayo imaging class predicts the age of kidney failure, tolvaptan (the only approved disease-slowing drug), the evidence for water and salt, brain aneurysm screening, whether children should be tested, and how huge kidneys are handled on dialysis and at transplant.
