After transplant
Why you must never miss an anti-rejection dose
Among the reasons a transplanted kidney fails, one is entirely avoidable: not taking the medicines properly.
- 1A study of why kidney transplants fail found that about half of rejection-related losses involved non-adherence — the most preventable cause of all.
- 2Anti-rejection drug levels must stay steady: take them at the same times every day and keep the timing relative to food consistent (always fasting or always after meals) so levels don't swing.
- 3Never reduce, stop or switch brands on your own; switching brand or formulation (immediate ↔ extended release) requires new drug-level checks arranged by your transplant team.
- 4Missed a dose? Don't double up. Rules differ by drug (some can be taken within a few hours, otherwise skipped) — ask in advance and write them down.
- 5On trough-level day, have blood drawn before the morning dose, or the number is meaningless. Use a pill organiser, phone alarms and a time-zone converter when travelling.
Nobody gets told off for asking "What do I do if I miss a dose?" Ask first, write it down, set the alarm.

|---|---|---|
| 0–5 | Pill organiser with one empty slot | Last night's tablet… forgotten. Does it matter? |
| 5–16 | Pie chart: ~half of rejection losses involve non-adherence | A study tracked why transplanted kidneys fail. Of the failures caused by rejection, about half involved not taking the medicines regularly. It is the most preventable cause. |
| 16–27 | Level curve: steady vs spiky; clock + plate | Anti-rejection drugs need steady blood levels. Same times every day, same timing relative to food — always fasting or always after meals — so levels don't swing. |
| 27–36 | Two different brand boxes + "levels must be rechecked" | Never reduce, stop or switch brands yourself. A different brand, or immediate to extended release, means new level checks arranged by your transplant team. |
| 36–46 | "Missed dose: don't double up", handwritten rule card | Missed one? Don't double up. Rules differ by drug — some can be taken within a few hours, otherwise skipped. Ask in advance and write it down. |
| 46–55 | Blood draw → then tablet; organiser, alarm, time-zone tool | On trough-level day, blood first, then the morning tablet, or the number is meaningless. Pill organiser, phone alarms and a time-zone converter help you never miss. |
| 55–60 | Site logo + URL + QR | Travelling across time zones? Beyond CKD has an anti-rejection dose-timing converter. |
