Diabetes & kidneys
Diabetes damages kidneys — but now there are drugs that help
Diabetes is the leading cause of dialysis in many countries — but the last few years brought medicines that protect the kidneys.
- 1Diabetes is the leading cause of new dialysis in the US and many other countries; long-term high glucose damages the kidney filters, usually silently at first.
- 2If you have diabetes, guidelines advise checking eGFR and urine albumin (UACR) at least once a year to catch damage early.
- 3Glucose targets are individual: an HbA1c target anywhere from <6.5% to <8.0% may be appropriate, depending on age, hypoglycaemia risk and other conditions.
- 4SGLT2 inhibitors: recommended for type 2 diabetes with CKD and eGFR ≥20 — they slow kidney decline and protect the heart.
- 5Metformin can be used at eGFR ≥30 (reduce the dose at 30–44); not advised below 30. Medicines must be adjusted as kidney function changes — ask your doctor to review regularly.
With diabetes, ask at your next visit: "Have I had a UACR this year? Am I a candidate for an SGLT2 inhibitor?"

| 0–5 | Glucose meter + kidney icon | Why does diabetes damage the kidneys — and is there anything that can stop it now? |
| 5–15 | Bar chart: #1 cause of dialysis "diabetes" | In the US and many countries, diabetes is the leading cause of new dialysis. High glucose damages the kidney filters for years, almost without symptoms. |
| 15–25 | Calendar tick "eGFR + UACR" | That is why guidelines advise people with diabetes to check eGFR and urine albumin, UACR, at least once a year. |
| 25–35 | HbA1c scale 6.5–8.0 | Glucose targets are individual. An HbA1c below 6.5 or below 8 may both be right, depending on age, hypoglycaemia risk and other illnesses. |
| 35–47 | Pill "SGLT2i" + kidney & heart shield | Here is the key: SGLT2 inhibitors. For type 2 diabetes with kidney disease and eGFR 20 or above, guidelines recommend them — they slow kidney decline and protect the heart. |
| 47–55 | Metformin + "eGFR ≥30" | Metformin is fine at eGFR 30 and above, reduced at 30 to 44, not advised below 30. When kidney function changes, medicines must change too. |
