Anti-rejection Medication Timing Converter

면역억제제 복용 시간 변환기

Pick your drug, enter the last dose and both time zones, and see what local time the next dose is due; add flight times to check whether a dose falls mid-flight.

The interval never changes. Next dose = time you actually took the last dose + standard interval (e.g. 12 h for tacrolimus). Time zones change the wall clock, not your body's clock.

Shift slowly. If you want a more convenient local time, move by 1–2 hours per day — not 8 hours at once.

Ask your team first. This tool only converts time. Any change to dose or schedule should be confirmed with your transplant team; trough levels are drawn before the dose.

기본값은 약물의 일반적인 간격(예: 타크로리무스 12시간). 처방이 다르면 숫자를 직접 입력(예: 8, 12, 24).

Flight times (optional)

Marks any dose times that fall during the flight.

Gradually shift to a convenient local time (optional)

e.g. 9:00 / 21:00 in Taiwan → 9:00 / 21:00 New York time.

Fill in the left side and press "Calculate next dose".

How this is calculated

  1. Dosing intervals come from FDA / EMA labels (every 12 h or every 24 h) and transplant-centre guidance (UHN, Duke, BC Transplant): same time every day, interval unchanged.
  2. All times are converted to UTC, the interval is added, and the result is displayed in the destination zone; daylight-saving is handled by the browser's Intl time-zone database.
  3. Gradual shifting follows travel-medicine convention — 1–2 hours per day to minimise level swings; each gap is constrained to standard interval ± shift.
  4. Missed-dose rules are from labels: immediate-release tacrolimus / cyclosporine can be taken if <6 h late; extended-release tacrolimus within 14–15 h; mycophenolate skipped if the next dose is <4 h away; never double.
