Blood pressure
Measure it wrong and the number is wrong
Kidney patients have a stricter blood-pressure target than most people — but most of us measure incorrectly at home. 5 things.
- 1High blood pressure is both a cause and a consequence of kidney disease; controlling it is one of the most important ways to protect kidney function.
- 2KDIGO 2021 suggests a systolic target below 120 mmHg with standardized measurement, when tolerated. That target applies only to a correctly taken reading.
- 3How to measure: sit quietly 5 minutes, back supported, feet flat, arm at heart level, no smoking/coffee/exercise in the previous 30 minutes, no talking; take two or more readings and average them.
- 4Home readings reflect real life better than clinic readings; measure at fixed times morning and evening and bring the log to your visit.
- 5Sodium under 2 g a day (about 5 g salt). For kidney patients with albuminuria, guidelines recommend an ACE inhibitor or ARB — don't stop it yourself because one reading looked normal.
Bring your home log to the next visit and ask: "What is my target, and am I measuring correctly?"

|---|---|---|
| 0–5 | Wrist monitor, legs crossed, talking | Is this how you take your blood pressure? Then the number is probably wrong. |
| 5–14 | Kidney and blood-vessel animation | High blood pressure is both a cause and a result of kidney disease. Controlling it is one of the most important things you can do for your kidneys. |
| 14–24 | "<120 mmHg (standardized measurement)" | KDIGO 2021 suggests a systolic target below 120 with standardized measurement. But that target only applies to a properly taken reading. |
| 24–38 | Correct posture: back supported, feet flat, arm at heart level | Sit quietly five minutes, back supported, feet flat, arm at heart level. No smoking or coffee in the previous thirty minutes. Don't talk. Take two or more readings and average. |
| 38–47 | Phone log of morning/evening readings | Home readings reflect real life better than the clinic. Measure morning and evening at fixed times and bring the log to your doctor. |
| 47–55 | Salt shaker "<5 g" + pill bag | Sodium under two grams a day — about five grams of salt. With albuminuria, guidelines recommend an ACE inhibitor or ARB. Don't stop it because one reading was normal. |
