Hemodialysis
Why fluid matters between dialysis sessions
The real cause of thirst isn't water — it's salt. 5 keys to fluid control on dialysis.
- 1When the kidneys can't remove water, extra fluid between sessions stays in the body: swelling, breathlessness, high blood pressure; removing it too fast on dialysis causes cramps and low blood pressure.
- 2Studies link larger interdialytic weight gain to higher cardiovascular mortality. A common unit target is no more than 4% of dry weight (about 2–2.5 kg); your own target is set by your dialysis team.
- 3Thirst is mainly driven by salt: the more sodium you eat, the thirstier you get. Cutting salt works better — and hurts less — than forcing yourself not to drink.
- 4"Fluid" is not just water: soup, congee, ice, jelly, watermelon and drinks all count; fruit and vegetables contain water too.
- 5Thirst tips: suck a small ice cube or lemon slice, use a small cup, chew sugar-free gum, and pour the day's allowance into one fixed bottle.
Your pre-dialysis weight is the best feedback. Ask the nurse: "What is my dry weight, and how much did I gain this time?"

| 0–5 | Dialysis-unit scale | Weigh-in before dialysis — up three kilos again. Why does fluid need controlling? |
| 5–16 | Swelling, breathlessness, high BP reading | When the kidneys can't remove water, everything you drink between sessions stays: swelling, breathlessness, high blood pressure. Take it off too fast on dialysis and you get cramps and a blood-pressure crash. |
| 16–27 | Line: more weight gain → higher risk; "<4% of dry weight" | Studies show the more weight you gain between sessions, the higher the cardiovascular risk. A common unit target is under four percent of dry weight, about two to two and a half kilos. Ask your team for yours. |
| 27–37 | Salt shaker → thirst icon | The real cause of thirst is salt. More sodium, more thirst. Cutting salt works better than forcing yourself not to drink — and hurts less. |
| 37–47 | Soup, congee, watermelon, ice, drinks in a row | Fluid isn't just water. Soup, congee, ice, jelly, watermelon, drinks — all of it counts. |
| 47–55 | Ice cube, lemon slice, small cup, fixed bottle | Thirst tips: a small ice cube, a lemon slice, a small cup, sugar-free gum, and the day's allowance in one fixed bottle. |
